Since the late 1990s, the same executive has led one of the country’s largest healthcare landlords through bankruptcies, a pandemic and repeated shifts in how care is delivered.

Cafaro began heading Ventas in 1999 and has been in the top job ever since, growing the healthcare real estate investment trust into one of the largest owners of senior housing, medical office and life sciences properties in the country. On the way, she became the longest-serving female CEO of a company in the S&P 500.

She started her tenure by spinning Ventas out of Vencor and forged her reputation during periods of stress. Ventas endured tenant bankruptcies in the 2000s, then faced significant pressure during the pandemic as senior housing occupancy plunged and operating costs soared. In response, Cafaro reshaped the portfolio, reducing exposure to skilled nursing and leaning more heavily into medical office and life sciences assets.

Recent years have brought cautious recovery. Senior housing demand has improved unevenly, but labor costs and interest rates remain persistent headwinds. Cafaro has emphasized balance-sheet strength and disciplined capital allocation, resisting the kind of aggressive growth favoring size over quality that left some operators exposed to changes in insurer reimbursements that can pepper the sector.

Cafaro is known for her direct style and long-term view. Her career has shown both the promise and fragility of healthcare real estate — an asset class tied as much to demographics as to execution.
